Underwent standard booking and classification at the San Mateo County Jail; her demeanor was described as cooperative, but nervous. The booking officer administered an intake screening form to Ms. Ramirez in which she answered "no" to all problem areas except for indicating she had scratched her neck area. She denied feeling suicidal or having a prior history of suicidal behavior. Ms. Ramirez also denied currently taking medication. Following completion of the booking and classification process, she was transported to the San Mateo County Women's Correctional Center WCC ; . Ms. Ramirez's confinement in the WCC initially appeared to be unremarkable. On March 13, she was sentenced to 210 days in jail on the pending charges. On March 16, Ms. Ramirez received a standard history and physical examination, and no medical problems were noted. Two days later on March 18, she requested Elavil and was referred to mental health staff. During the afternoon of March 22, Ms. Ramirez threatened suicide and was subsequently seen by a jail nurse. She "contracted for safety" until assessed by mental health staff, and was given a three-day supply of Ataarx to help her sleep and relieve symptoms of anxiety. On March 25, Ms. Ramirez was assessed by mental health staff. She denied feeling suicidal or having a prior history of suicidal behavior, although she expressed anxiety and felt either "embarrassed, ashamed or humiliated." She informed the clinician that she only threatened suicidal over the weekend because she wanted medication for anxiety. Because she appeared to be deceptive, and the mental health clinician was reluctant to recommend psychotropic medication to an inmate with an alleged history of drug abuse, Ms. Ramirez was instructed to ask her family to bring in the medication. During the late evening of March 28, Ms. Ramirez was seen by medical staff after complaining that she had hit her forehead in the shower. She again denied any suicidal ideation, claimed the injury was an accident, and requested to be transferred to the honor camp because she did not get along with several inmates in the dorm. Per the request of correctional staff, Ms. Ramirez was placed on a referral list to see mental health staff. She was seen by mental health staff on April 1 and repeated her earlier request for medication. She was again told to ask her family to bring in the medication. Sometime before 7: 00pm on April 6, 2003, Ms. Ramirez was observed by several other inmates in her housing unit to be walking toward the bathroom and shower area with a sheet loosely draped around her shoulders. A few of the inmates observed her to be crying. An officer, who had not observed Ms. Ramirez walk into the shower area or saw her crying, then escorted several inmates out of the dorm for recreation. Upon returning at approximately 8: 00pm, two of the inmates entered the shower area and observed Ms. Ramirez hanging from a bed sheet that was attached to the shower rod. They began to yell for assistance, and an officer arrived at the shower stall and, with assistance from another inmate, removed the noose from around the victim's neck and laid her on the floor. A second correctional officer arrived shortly thereafter, and cardiopulmonary resuscitation CPR ; was initiated by the officers. Emergency medical services personnel arrived within several minutes and continued CPR efforts. Their efforts were unsuccessful and Angela Ramirez was subsequently pronounced dead.

Augmentative and Alternative Communication AAC ; devices are available to help these individuals communicate. However, AAC users have differing sets of capabilities and it is important to select the AAC system from the wide range of available systems that will allow them to make the most effective use of their capabilities. At present, to the extent that variations in systems can be made available, they are produced either by a computer professional who must adapt them at considerable expense or by someone with quite other concerns, for example a busy speech therapist, who must learn enough about the system to tailor it to the needs of particular individuals. Researchers at the University of Abertay, Dundee, are currently developing an alternative solution that combines expert systems with an object approach to developing AAC devices. Portia explained how an expert system can collect information about a particular AAC user's requirements and produce a specification for that user's AAC system. This specification is then used to select system features from a tool kit of such features, each represented as an object. These components are then combined to produce the implementation that is tailored to the user's needs and actoplus.
